Trellis is seeking highly motivated Specialty Pharmacist who is well-versed in dispensing specialty medications and eager to elevate their practice. The Specialty Pharmacist is part of the On-Site Operations Team and is the frontline team member responsible for the dispensing and delivery of specialty pharmacy services as part of an integrated care team. The Specialty Pharmacist’s role is to own the dispensing operations for each patient that we serve. To ensure the best clinical outcomes, this person should also be prepared to assist with other tasks that their patients require, including prior authorizations and financial assistance. In this role, you will also be responsible for overseeing the operations of the primary location of the specialty pharmacy in compliance with accreditation standards.

You would be responsible for:

Dispensing / Regulatory duties

  • Performing order entry of prescriptions into the pharmacy management systems (Dispensing, POS, etc.) and making sure claims are appropriately adjudicated
  • Overseeing implementation of blister packing and cold chain policies and procedures
  • Ensuring specialty medications delivery route and coordination is appropriate (e.g. by mail, courier or pickup)
  • Performing clinical checks on prescriptions for correct dosage, indication, drug interactions, end-organ function, etc.
  • Maintaining own professional and specialized competency through continued education
  • Meeting Trellis Rx standards in the following areas:
    • Conforming to Trellis Rx standards of performance and conduct, including those pertaining to patient rights, so that the best possible customer service and patient care may be provided.
    • Utilizing Trellis Rx values as the basis for decision-making and to facilitate the mission of Trellis Rx and its customers
    • Respecting and following all customer policies are procedures
  • Assisting in the development and revision of policies and procedures
  • In all activities, complying with accreditation, legal and regulatory requirements

Patient Management responsibilities

  • Establishing great working relationships with health system staff and physicians in clinics and patient populations served by Specialty Pharmacy
  • Leading work in the Specialty Pharmacy Operations site, owning operational outcomes and associated KPIs (as set), and ensuring that the patient/Trellis Rx engagement is appropriate and consistent with customer preferences. This includes:
    • Ensuring that every patient is dispensed a therapeutically appropriate medication in a manner that meets the requirements of the medication and the needs of the patient
    • Showing patients, through action, the benefits of using the customer’s specialty pharmacy services, without impeding the patient choice of pharmacy
    • Leading clinical follow-up with patients per defined protocols – through outbound phone calls– to ensure patients are adherent to therapy and any issues are identified and resolved expediently
    • Accepting inbound calls from patients, and sharing in on-call coverage to ensure 24/7 patient support
    • Appropriately addressing clinical questions, triaging as appropriate
  • Partnering with physicians and pharmacists who refer patients to the specialty pharmacy service to ensure therapies selected are evidence-based
  • Participating in leadership and clinic staff meetings to help explain the Trellis Rx model
  • Assisting the site General Manager with recruiting and training of on-site Pharmacy Liaisons and Clinical Pharmacy Specialists
  • Assisting with administrative, compliance-related or other tasks to ensure patients in the program receive unmatchable service
  • Ensuring the Pharmacy Operations location is adherent to the standards required to obtain accreditation through identified accrediting bodies

Oversight responsibilities

  • Developing and implementing best-in-class operational, clinical and research protocols for specific disease states, to markedly improve patient outcomes
  • Assessing and developing medication regimens that are also cost-effective and affordable by the patient
  • Owning quality improvement to drive key operational outcome indicators at a patient and population level
  • Developing training materials to help onboard Clinical and Operational Pharmacy Specialists and on-site Pharmacy Liaisons
  • Developing and delivering workshops and development seminars for client-based pharmacy specialists and leadership groups
  • Representing Trellis Rx at conferences and workshops
  • Other projects and tasks within the scope of pharmacy practice as assigned

We are excited to speak with someone who has:

  • Bachelor’s Degree in Pharmacy from an accredited college of pharmacy. Advanced degree (e.g., PharmD, M.S.) preferred
  • Experience as a Clinical Pharmacist in a Specialty Pharmacy preferred, with demonstrated experience in pharmacy dispensing
  • The ability to work across specialties
  • The energy and willingness to be hands-on and do whatever is required to provide the best care to our patients
  • The ability to start from scratch, execute quickly and accurately without pre-defined processes and protocols, and an interest in defining such processes and protocols for the future
  • Strong people skills and the ability to work with teams across customers
  • The ability to communicate effectively at all levels inside and outside the organization
  • Proficiency in MS Office including Word, Excel, PowerPoint, and Outlook Calendar
  • Proficiency in multiple EMR systems such as Cerner, EPIC or Meditech
  • Proficiency with dispensing systems such as WAM, QS1, RX30, Computer Rx etc.

And bonus points if you have a couple of these, too:

  • Experience working for a specialty pharmacy within a health system environment is a huge plus
  • Licenses to practice in multiple states and/or the willingness to obtain additional state licenses
