In Scioto County, Ohio, 13.5% of adults are living with diabetes, surpassing the national rate of 8.5% and the Ohio rate of approximately 9.9%1. Studies show that consistent medication utilization improves diabetes management, yet adherence rates nationally vary significantly.
The purpose of this investigation was to determine the impact that offering integrated chronic disease management (CDM) pharmacy services had on improving patient outcomes at Southern Ohio Medical Center.
