An estimated 6.2 million American adults are living with heart failure (HF), placing a significant financial burden on patients and healthcare resources.
The purpose of this investigation was to determine the impact embedding a clinical specialty pharmacist within a health system’s cardiology clinic had on decreasing hospitalizations and readmissions through reducing barriers to care among HF patients.
