ATLANTA, GA, December 6 — Trellis Rx today announced it has been selected by Stamford Health, a non-profit independent healthcare system in Stamford, Connecticut, to enhance the medication experience for their patients by launching an in-house retail and specialty pharmacy service. The Stamford Health Pharmacy will integrate highly trained pharmacists and pharmacy liaisons into care teams to ensure patients receive the coordinated, high-touch support needed to improve medication adherence and achieve their best health.

In addition to providing medication to patients upon discharge from the hospital, the Stamford Health Pharmacy will offer specialty pharmacy services to patients living with chronic conditions such as cancer, rheumatoid arthritis, and HIV. The medications used to treat these conditions are often challenging for patients to access and manage due to high out-of-pocket expenses, significant side effects, and other barriers. As a result, patients struggle to adhere to their treatment regimens without the right level of clinical, financial, and emotional support.

Working with Trellis Rx, Stamford Health will provide patient-centered specialty pharmacy services to address these barriers. The services will deploy highly trained specialty pharmacists and pharmacy liaisons in specific specialty practices to provide patients with local, personalized care. In addition to ensuring patients can access their therapies and evaluating medication appropriateness, these experts will provide ongoing counseling and education to promote adherence. They will also proactively monitor therapy effectiveness and collaborate with providers to promote high-level clinical outcomes.
